WCS Healthcare has an immediate opening for multiple Case Managers to coordinate care within a large Social Services Agency in the Bronx, NY.  

Compensation : $24 - $25

Job Function
  • Ensure that each client is receiving appropriate and effective case management services
  • Conduct comprehensive assessments and develop service plans that identify long and short-term goals that the client agrees with.
  • Conduct in-person contacts (home visits/office visits) as specified in the service plan(s) with a minimum of one home visit per month and one contact per week.
  • Provide crisis intervention to clients and their families as needed.
  • Arrange support and home health care services as necessary, making referrals to medical, home care, mental health and/or substance abuse services
  • Ensure that clients pay the balance of their rent, telephone and utility bills.
  • Provide counseling and orientation regarding tenant’s rights and responsibilities to maintain and retain housing.

Job Requirements:

  • Associate’s Degree in human services or related field
  • Two (2) years’ administrative work experience
  • Experience with HIV/AIDS afflicted individuals, chronic mental illness, substance abuse individuals and their families a plus
  • Bilingual skills in English/Spanish a plus
