Job Function:
Direct the medical care and treatment of patients and have primary medical responsibility in overseeing and supporting the care rendered by other members of the interdisciplinary care team
Monday to Friday 8-5pm schedule (40 Hour work week)
Will be based out of clinic but may need to go into the field and go to patients homes, but there are home aides that do this
Collaborates with site NP community-based primary care providers on the shared population as well as social workers and dieticians
Will have own patient panel as Primary Care Provider and participates in direct medical care in the Diagnostic and Treatment Center providing med management, patient assessments, and examinations
Will be overseeing clinical staff (NPs and MAs)
Participates in developing preventive health screening, immunization programs, professional community outreach, quality assurance, etc.

NY State license, DEA, CSR (active or applied preferred) but not required
Graduate of an Accredited School of Medicine
Residency in Internal Medicine, Family Practice, or Fellowship and/or experience in Geriatrics.
Board Certification in Internal Medicine, Family Practice or Geriatrics
Fluency in Bengali, Hindi, Urdu or Punjabi preferred but not required
