The largest not-for-profit Program of All-Inclusive Care for the Elderly (PACE) in the nation is looking for a Nurse Practitioner (ANP or FNP) for their location in Brooklyn.

Job Function:
Will perform annual wellness assessments on all assigned site community participants and reflects an understanding of the complexity and multiple comorbidities of the elderly
Monday to Friday 8:30-5pm schedule (37.5 hour work week)
Will be based out of clinic but may need to go into the field and conduct home visits, but there are home aides that do this
Responsible for reviewing patients' medical history every 6 months and assesses those that were hospitalized or sent to ED
Will be responsible for identifying active diagnoses and chronic problems or conditions to be used in Care Management and active medical management of treatment and designed interventions
Responsible for supervising the care plan and delivery by each provider

NY State license, DEA, CSR (active or applied preferred) but not required
Masters as an Adult or Family Nurse Practitioner
Board Certification Required
Previous experience in geriatrics, home care or care management required
